KPI INVESTIGATION QUESTIONNAIRE

Prior to selecting the following phenomena that best describes the experiences that you are having, KPI urges you to explain by any means possible (debunk), how certain phenomena have occurred. When all sensible explanations have been thoroughly considered and eliminated as the cause, only then can the phenomena be classified as Cause Undetermined or Unexplainable Phenomena.
